STUDENT RECORDS SPECIALIST
To register your child at Piñon Unified School District #4, please stop by the school office and complete a registration packet. You will need to be prepared to provide emergency contact and medical information.
You will also need to bring the following original documents to our office. We’ll make copies and return the originals back to you.
Please Bring the Required Documents:
Report Card/Transcript with grade placement for SY 21-22
Social Security Card
Certificate of Indian Blood
Updated Immunization Record
Current Physical Address Document
Guardianship Documents (if applicable)
New Student Enrollment for Kindergarten to 12th Grade

Returning Enrollment for Kindergarten to 12th Grade
If you wish to enroll your RETURNING student online, you must create a Parent PowerSchool account. If you already have a Parent PowerSchool account, but you forgot your password, please contact the I.T. Department for a password reset.
PAPER ENROLLMENT PACKET
ELEMENTARY SCHOOL - KDG -5TH GRADE
MIDDLE SCHOOL - 6TH - 8TH
HIGH SCHOOL - 9TH - 12TH
Upon opening the Fillable PDF Registration Packet, please download the file to the computer. When you are done completing the forms, please save the file. Go to File - Save As - Name the file with Parent's name. Email the save file to the selected school.
To send in your supporting documents for New Student Enrollment, Proof of Residency, Updated Immunization, CIB, SS Card, Birth Certificate, you may email them to the school's Records Specialist.

Arizona state law requires children attending school to provide proof of immunization at the time of registration. Subject to state policy A.R.S. 15-873, we may deny attendance of a student not meeting state immunization or exemption requirements.
